Conditions for Using and Processing Personal Data

Thank you for using the services on our website. The services are provided by Eye Centre Prague, a.s., CID No 26169231, registered office: Holešovice 1637, Prague 7, 170 00.

The controller of your personal data is Eye Centre Prague, a.s., CID No 26169231, registered office: Holešovice 1637, Prague 7, 170 00 (hereinafter the "Controller"), pursuant to Regulation (EU) No of the European Parliament and of the Council on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) (hereinafter the "Regulation").

THE DATA WE PROCESS AND OUR REASONS FOR PROCESSING THEM 

1) Website visits - cookies:

We process cookies when you visit our website. Some of these files help the website function properly and link your activities while you browse the website.

The collection of cookies for this purpose can be considered personal data processing. This processing is allowed on the basis of a legal reason - the legitimate interest of the Controllers, and is enabled by Art. 6 par. 1 point f) of Regulation (EU) No 2016/679 of the European Parliament and of the Council on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) (hereinafter the "Regulation").

The website can also be used in a mode that does not allow the collection of data on the behaviour of website visitors - this mode can either be set in your browser settings, or you can object to this collection based on the legitimate interest of the Controllers pursuant to Art. 21 of the Regulation by writing to recepce@ocp.cz. Your objection will be evaluated without undue delay, no later than within 30 days of its receipt. Cookies necessary for the website's functioning will only be kept for the time necessary for the website to function.

If you object to the processing of technical cookies necessary for the functioning of the website, the full functionality and compatibility of the website cannot be guaranteed.

Cookies collected for the purpose of measuring web traffic and creating statistics on the traffic and behaviour of web visitors are assessed as a collective whole and in pseudonymized form.

We also transfer data about your web behaviour to other websites and social media sites for the purpose of targeted advertising.

Tracking conversions in Google Ads
 
We use conversion tracking in Google Ads, which includes the functionality of extended conversion to increase the accuracy of tracked conversions. Marketing data about our website visitors are transferred to Google to help us further improve the relevance of conversion tracking. Before the data are sent, they are encrypted for maximum protection. Google may pair them with its database precisely for more accurate conversion tracking. The information about our website visitors that we pass on to Google includes data you enter into forms on our website, such as an order or contact form. When we transfer personal data, Google becomes another data controller; otherwise we use our own controller. 2) Sending a contact form or booking a consultation

The following personal data are processed when you use a contact form:

  • your full name;
  • your email address;
  • your phone number;
  • your age;
  • your home address;
  • your IP address;
  • the health problems you described.

The processing of your personal data – full name, telephone number and email address - is permitted by Art. 6 par. 1 point b) of the Regulation - processing is necessary for implementation measures adopted prior to the conclusion of the contract at the request of the data subject. These personal data need to be processed to enable communication between you and the Controller.

The processing of personal data related to health problems you have described is the processing of a special category of personal data. This processing is necessary for recommending suitable services offered by the Controller, but it is completely voluntary. Your explicit CONSENT is required for this processing, which you can grant us by clicking on "I have read/I consent to the processing of personal data". This processing is allowed under Art. 9 par. 2 point a) of the Regulation.

Controllers process personal data in the following manner:
We will respond to your request in an email.
Your IP address is stored in order to recognize spamming activity and to defend ourselves against this activity by blocking access for these IP addresses. This processing is allowed under Art. 6 par. 1 point f) of the Regulation - legitimate interest of the Controllers. You have the right to object to the processing of your IP address at recepce@ocp.cz

This data will be processed by the Controller for the duration of your mutual negotiations, but for no longer than 3 years from the last communication, unless a contract is concluded. In this case, the communication will be archived for a period of 10 years from the last provided service in order to defend the rights of the Controllers and due to the valid legislation in the field of health services.

 
3) Sending a question to the online consultation room

When you send a question to the online consultation room or write a reference, we process the following data:

  • your name or username;
  • your email address;
  • your IP address;
  • other personal data that you voluntarily provide in your message

This processing is allowed under Art. 6 par. 1 point a) of the Regulation - your consent. The processing of personal data related to health problems you have described is the processing of a special category of personal data. Provision of this data is completely voluntary. Your explicit CONSENT is required for this processing, which you can grant us by clicking on "I have read/I consent to the processing of personal data". This processing is allowed under Art. 9 par. 2 point a) of the Regulation.

All these personal data will be processed by Controllers for 6 years after your question or reference is published.

Your IP address is stored in order to recognize spamming activity and to defend ourselves against this activity by blocking access for these IP addresses. This processing is allowed under Art. 6 par. 1 point f) of the Regulation - legitimate interest of the Controllers. You have the right to object to the processing of your IP address at recepce@ocp.cz.


4) Ordering a newsletter or guides for download

When you subscribe to our newsletter or download our PDF guide, we process the following data:

  • your email address;
  • your IP address;

We will process your email address for a period of 3 years after you grant your consent to receiving commercial messages. This processing is allowed under Art. 6 par. 1 point a) of the Regulation. You can withdraw your consent at any time at reception@ocp.cz, or by unsubscribing in the commercial message.

If you have ordered a downloadable guide, we will process your email address for the time necessary to process your order, but for no longer than 3 years. This processing is allowed under Art. 6 par. 1 point b) of the Regulation.

Your IP address is stored in order to recognize spamming activity and to defend ourselves against this activity by blocking access for these IP addresses. This processing is allowed under Art. 6 par. 1 point f) of the Regulation - legitimate interest of the Controllers. You have the right to object to the processing of your IP address at recepce@ocp.cz.

 
5) "Rate us" questionnaire

When you fill out and send the "Rate our work" questionnaire, we process the following data:

  • name
  • surname
  • other personal data that you voluntarily provide in the questionnaire/attachment

This processing is allowed under Art. 6 par. 1 point a) of the Regulation - your consent. The processing of personal data related to health problems you have described is the processing of a special category of personal data. Provision of this data is completely voluntary. Your explicit CONSENT is required for this processing, which you can grant us by clicking on "I have read/consent to the processing of personal data". This processing is allowed under Art. 9 par. 2 point a) of the Regulation.
 
All these personal data will be processed by Controllers for 6 years after your question or reference is published.

WHO PROCESSES THE DATA?

Your personal data are processed by the Controller, but the following processors can also process data for the Controller:

1. Software/IT providers, service agencies and other cooperating entities:

  • INDIGO Multimedia, s.r.o. (website, web hosting and CRM system administrator)
  • Atweb Consulting, s.r.o (marketing agency)

2. Any other providers of processing software, services and applications that are not currently used by the company.

 

YOUR OPTIONS AND RIGHTS

Please note that you have the following rights under the Regulation:

  • the right to withdraw your consent at any time in an email or letter sent to the company's address: recepce@ocp.cz;  
  • the right to request information about the type of data we process;
  • the right to request a copy of your processed personal data;
  • the right to request access to this data to update or rectify them, or request restriction of their processing;
  • the right to have your personal data erased;
  • the right to object to the processing of data based on legitimate interest;
  • the right to data portability in the case of data processed in an automated manner on the basis of your consent or to perform a contract;
  • if you have any doubts about the legality of personal data processing, you can file a complaint with the Office for Personal Data Protection, or file a motion in court.

Privacy Policy

The service provider makes sure that all personal data is protected in accordance with applicable laws and secured against misuse or unauthorised access by third parties.

 
By accepting these Terms of Use, you acknowledge that your data may be further indexed by web search engines due to the fact that one of the website's services consists in the publication and dissemination of information. This Policy only applies to services provided by the website provider Eye Centre Prague, not to services, personal data and other information provided by third parties published on websites that can be visited via links on the service provider's website (i.e. links to websites of third parties).
